Clinical Study of Malignant Hypertension

Abstract
|
|
|
Clinical observation was investigated in 115 cases of maligant hypertension diagnosed according to a new criteria and following result were. obtained.
1) The prevalence rate of malignant hypertension among essential hypertension was 4.7 percent.
2) Among 115 cases of malignant hypertension 74 cases 64 percent were male and 41 cases 35 percent were female. The ratio of male to female was 1.9 : 1. Malignantt hypertension was observed most frequently in 5059, .40^49 and 60-69 years of age in this order.
3) The most frequently observed duration of known hypertension 38.9 percent was 6^-¢¥10 years and the next frequently observed duration 27.3 percent was 3-5 years.
4) The highest percentage of malignant hypertension had syotolic blood pressure between 210 and 229 mmHg and diastolic blood pressure between 130 Wand 149 mmHg.
w 5) -79. 3 ¢¥percent of the patients were found that their blood pressure were abnormaly high when they were examined with related symptoms. 12.7 percent of malignant hypertension was first found when they developed complications such as cerebrovascular accident. This indicates that the malignant hypertension is a- disease with rapid progress and accelerated deterioration.
6) The status of treatment in malignant hypertension was as follows: intermittent treatment 43.8 percent, no, treatment 31.5 percent and conti:-nuous treatment 16.9 percent.
7) According .to the degree of funduscopic ¢¥abnor-mality¡Æby¢¥ K=W classification `grade Y was ¢¥54.8 Percent and grade 1 1 . was 45.2 percent.
8) Proteinuria was observed - in 92.8 percent of malignant hypertension.
9) Seurm Cholesterol was abnormally high in 80.9 percent of malignant hypertension.
10) 96.5 percent of malignant hypertension revealed electrocardiographic abnormalities and. left ventricle hypertrophy was observed in 91.3 percent of malignant hypertension.
11) The most frequeutly observed complication among malignant hypertension was renal insufficiency occupying 50.4 percent, the next common complications were cerebrovascular accident (25.2 percent) and congestive heart failure(22.6 percent).
12) The plasma renin activity, (PRA) in malignant hypertension was high in 8 cases among 13 cases examined, normal in 4 cases and .low in 1 case.
